Signs and symptoms

Is it low T, or something else?

Testosterone naturally declines with age, but some men drop low enough to feel it every day:

  • Low sex drive or erectile changes
  • Fatigue that sleep doesn't fix
  • Loss of muscle and strength, or more belly fat
  • Low mood, irritability, or trouble focusing
  • Lower motivation or drive

These overlap with poor sleep, stress, thyroid problems, and depression. That's why we test before we treat.

Diagnosis

How we diagnose low T

A single lab value doesn't tell the whole story. A proper workup includes:

  • Two early-morning testosterone tests on different days
  • LH and FSH to show where the problem starts, which guides treatment
  • Supporting labs: blood count, PSA when appropriate, thyroid, blood sugar, lipids
  • A physician visit covering symptoms, medications, sleep, and whether you want children in the future

Labs are drawn at our clinic, and labs and medically necessary visits are often covered by insurance.

Low T often has a cause we can fix

Before talking about medication, we look for what's pulling your testosterone down.

Excess weight

Body fat converts testosterone to estrogen and lowers the signal to make more. Losing weight often raises testosterone on its own.

Sleep apnea

Most testosterone is made during sleep. Untreated sleep apnea is a common, often missed cause of low T and fatigue. We offer home sleep testing.

Medications

Opioids, steroids, and some other drugs can lower testosterone.

Alcohol and stress

Heavy drinking and chronic stress both suppress testosterone production.

Other conditions

Thyroid disease, diabetes, or problems with the pituitary gland can all play a role.

The foundation

Diet and exercise come first, whatever you choose

For some men, these changes alone bring testosterone back into a healthy range. For others, they make treatment work better at a lower dose.

Strength training

Two to four times a week, plus regular walking or cardio.

A healthier weight

With help from our medical weight loss program if you want it.

7 to 9 hours of sleep

And treatment of sleep apnea if you have it.

Whole-food diet

Protein and vegetables, less alcohol, fewer ultra-processed foods.

Your options: testosterone vs. clomiphene vs. enclomiphene

There's no single best treatment for low T. The right choice depends on your labs, your symptoms, and especially whether you want to have children.

Lifestyle changes

How it works: Removes what's suppressing your own testosterone

FDA status: Not applicable

Form: Diet, exercise, sleep, weight loss

Fertility: Neutral or helpful

Best fit for: Everyone, as the foundation

Side effects: None

Monitoring: Weight, labs as needed

Testosterone therapy (TRT)

How it works: Replaces testosterone directly

FDA status: FDA-approved for men with low T from a medical cause

Form: Injections, gels, and other forms

Fertility: Lowers sperm production and can cause infertility

Best fit for: Men with confirmed low T who aren't planning to have children

Side effects: High red blood cell count, acne, breast tenderness, testicular shrinkage

Monitoring: Testosterone, blood count, and PSA on a schedule

Clomiphene

How it works: Pill that signals your body to make more of its own testosterone

FDA status: FDA-approved for women's fertility; use in men is off-label

Form: Daily or every-other-day pill

Fertility: Generally preserves sperm production

Best fit for: Men who want to protect fertility, when the testes still work normally

Side effects: Mood changes, breast tenderness, rare vision changes

Monitoring: Testosterone and estrogen levels

Enclomiphene

How it works: A purified form of one part of clomiphene; works the same way

FDA status: Not FDA-approved. Only available from compounding pharmacies

Form: Daily pill

Fertility: Small studies suggest it preserves sperm production

Best fit for: Men who want to protect fertility and understand the limited evidence

Side effects: Headache, hot flashes, nausea; long-term safety not well studied

Monitoring: Testosterone and estrogen levels

What to know about each

The honest pros and cons, so you can make the choice that fits your goals.

Testosterone

Testosterone therapy

The most studied option, and it raises levels reliably. A large 2023 study (TRAVERSE) did not find a higher risk of heart attack or stroke compared with placebo in men with low T.

Because outside testosterone shuts down your own production, it isn't a good choice if you're trying to conceive. Testosterone is a controlled substance and requires regular follow-up.

Clomiphene

Clomiphene

Helps your body make more of its own testosterone instead of replacing it. It's a generic pill that has been used for decades.

It's prescribed off-label for men, which is common and legal, but it means the FDA hasn't reviewed it specifically for this use.

Enclomiphene

Enclomiphene

One of the two forms (isomers) that make up clomiphene. It is not FDA-approved, and compounded versions are not reviewed by the FDA for safety, effectiveness, or quality.

Small studies show it raises testosterone and may protect sperm counts, but long-term data is limited. We'll walk you through the trade-offs honestly.

Physician-led care

Real doctors, not a hormone clinic

Online clinics and med spas often start with a prescription. We start with you: your labs, your sleep, your weight, your goals. Your care is led by board certified doctors and our Austin primary care team, who already know the rest of your health.

That means safer treatment, fewer surprises, and a plan that fits your whole life.

Many guidelines use a morning level below about 300 ng/dL, confirmed on two separate tests, along with symptoms. Your physician will look at the full picture, not one number.

Sometimes. Weight loss, strength training, better sleep, and treating sleep apnea can raise testosterone meaningfully, especially in men carrying extra weight.

It can. Testosterone therapy lowers sperm production. If you want children now or in the future, tell us. Options like clomiphene may be a better fit.

Enclomiphene isolates the part of clomiphene thought to do most of the work, which may mean fewer side effects. But it isn’t FDA-approved, and the research is still limited. We’ll help you weigh it against clomiphene and testosterone therapy.

We accept most major insurance plans for labs and medically necessary visits. Some program fees and medications are self-pay.

Your first visit and labs are in person at our Austin clinic. Many follow-ups can be done by video.
